【問卷內容】

一、活動主旨：

為鼓勵大一新生認識本校e-portfolio（學生學習歷程系統），規劃有獎徵答活動。期盼藉由此活動的辦理，

讓學生從新生入學開始即認識e-portfolio平台的架構項目，增進學生對於系統的熟悉度。

二、參加對象：大一新生

三、活動時間：2025/9/26~10/24

四、活動方式：   

★請登入有獎徵答問卷網址 (請點我) 填答本校e-portfolio（學生學習歷程系統）相關問題(共5題)；

    或由校園入口→活動報名表「大一新生e-portfolio知多少？學生學習歷程系統有獎徵答活動」，完成有獎徵答題目。

★凡參加且全數答對者，即可享「參加獎」抽獎活動

五、活動獎勵：

參加獎：e-portfolio有獎徵答抽獎活動獎勵禮卷100元(共30名)。

六、獲獎名單公佈：

2025年10月27日下午五點前以學校e-mail通知獲獎者，並於圖書館網頁公布中獎名單。

七、領獎期限：

請於2025年11月28日前至圖書館兌換完畢(請攜帶學生證)，逾期視同放棄。

Dear Freshmen,

We welcome your active participation in the「E-portfolio Prize Quiz Contest for Freshmen」.

一、Purpose：

To encourage freshmen to learn about the e-portfolio system, a quiz contest with prizes is organized. This activity aims to help students understand the platform’s structure from the start of their studies and enhance their familiarity with the system.

二、Participants：Freshmen

三、Schedule：2025/9/26~2025/10/24

四、Format：

★Please log in to the prize quiz survey website (Click Here) to answer questions related to our school's e-portfolio (5 questions in total).

★Anyone who participates and answers all questions correctly will be eligible for the Participation Prize raffle.

五、Participation Prize：

e-portfolio Prize quiz reward voucher of 100 TWD, a total of 30 recipients.

六、Winner Notification：

Winners will be notified via school email by 5:00 PM on October 27

七、Deadline for Claiming the Prize：

Please claim your prize at the library by November 28 (please bring your student ID card).

Failure to claim within the deadline will be considered as forfeit.
