When to Report: Upon discovering or becoming aware of any suspected theft, leakage, alteration, damage, loss, misdelivery, mistransmission, or other infringement of personal data, appropriate measures shall be taken immediately to prevent further damage, and the unit supervisor and the University's Personal Data Protection Contact shall be notified.

Reporting Procedure: Upon discovering or becoming aware of a suspected personal data breach incident, appropriate measures shall be taken immediately to prevent further damage, and within 36 hours the "Personal Data Breach Incident Report and Record Form" shall be completed and submitted to the unit supervisor and the University's Personal Data Protection Contact.Once confirmed as a personal data breach incident, the University will report it to the Ministry of Education within 72 hours of discovery.
